Finding common ground between members is a challenge faced by any international organization, and the International Hospital Federation (IHF) is no exception. We are all part of a global economy where interdependence is a reality. The current global financial and economic crisis is the worst of its kind experienced in a long time. The major threat from the H1N1 flu epidemic affecting most countries remains upper most in all our minds. But while it is obvious that we all are in the same world, each country and individual is nevertheless driven by differences in environment and interests. Having to face very specific challenges related to our respective environments drives each one to focus on 'local' issues, and to pay little attention to what others are doing across the world. There is also a stronger recognition of the peculiarity of each country linked to its history and culture. No one size can fit all! In the short term focusing on 'local' issues pays better than looking out to the horizon to determine how to improve performance. Such an approach, however, limits the response capability to immediate challenges alone and denies the organization the possibility to move to the position where outcomes can be optimized. This latter goal that requires consideration of the broader picture is necessary as it provides the opportunity to anticipate would-be global dominant trends as well as learn from the success and failures of others.
